A medical capsule containing I131 was administered to an adolescent patient within the nuclear medicine department. It was discovered that, instead of swallowing the capsule as instructed, the patient spat it out and was hiding it in their hand. This led to an extensive contamination of the patient's hand, clothes, and chair. The immediate area was also contaminated, and the department was closed for decontamination.
