A package containing an industrial gamma radiography exposure device, incorporating depleted uranium shielding, housing an iridium-192 radiography source of activity 0.73 TBq(approx.) was being transported from Lakhimpur to Digboi in the Assam State of India. The package was taken by a radiographer in a passenger bus in contravention to the regulatory requirements. The bus broke down on the way and another bus was organised for all the passengers to continue the journey. Upon reaching the destination, the radiographer noticed that the package containing the source was missing.Efforts are on to locate the package.
